Each membership includes the affiliated provider consultation, prescribed treatment, injection supplies when applicable, standard shipping, and routine refill processing. Laboratory testing is not included unless specifically stated. Prescriptions are issued only when clinically appropriate.

Bundle Rules
Bundle discounts apply only while 2 or 3 eligible monthly memberships remain active at the same time.
Prepaid multi-month memberships do not receive an additional monthly bundle discount.
Complete LongevityCare does not receive an additional discount because it is already a four-treatment package.
Women's HormoneCare is billed every 3 months and is not automatically included in the monthly bundle structure.
If one membership is cancelled, the remaining membership or memberships return to their regular standalone pricing on the next billing date.
Patients may not exchange unused medications or treatments for another product, service, credit, or refund.
Each treatment remains subject to clinical eligibility and ongoing provider approval.

Provider-guided hormone therapy for eligible women. Treatment may include estradiol (estrogen), progesterone, Bi-Est (biestrogen), and/or DHEA when clinically appropriate. Provider review of ordered laboratory results is included. If laboratory testing is ordered, laboratory fees are paid separately by the patient directly to the laboratory.

Complete LongevityCare combines NAD+, sermorelin, glutathione, and B12 injections in one all-inclusive membership.
